• First and last name of patient
• Date of birth 
• Date of appointment
• Whether you  wish to CONFIRM, CANCEL OR REBOOK. 

If you are rebooking, someone will contact you with a new appointment. 

Due to our long waitlist, rebooked appointments may result in a delay in being seen for consultation.
